Your first visit to Shohet Ear Associates establishes a vital foundation for our relationship with you. Prior to your first visit with us, we’d like to make sure you are as prepared and comfortable as possible.
New Patients
In order to save some time at your first visit, we ask that you please download and complete the New Patient Information Form and the Patient Health Survey Form listed below.
There are two ways to complete the forms:
- Fill out the forms directly on your computer (there are fillable fields) and then save and email the forms to us at forms@eardoctor.org
OR - Print out the forms and complete by hand, and then bring them to your appointment
*We also have copies of the forms in our office that you can complete when you check in, if that’s more convenient for you.
Returning Patients
If you are an existing patient that we haven’t seen in over a year, please download and complete the Annual Update Form below before coming in for your appointment. If you have tinnitus, ringing in ear, dizziness, vertigo or poor balance, please complete the additional forms listed below.

What to Expect at your First Visit
Being well-prepared for your appointment will ensure that our medical doctors and audiologists have all of the necessary information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. You can also educate yourself on your symptoms by reviewing the content on our website under ‘Conditions & Treatments’. Also, take some time to review our team bios so you can familiarize yourself with our medical doctors and audiologists.
During your first visit, we make sure to obtain important background information, like your medical history and give you time to get to know your medical doctor or audiologist. Most initial evaluations take approximately one hour. More complex or emergency (acute) type evaluations can take up to two hours. Please allow for sufficient time for us to complete the examination and provide the best care for you.
