Read This Before Your First Visit
Charges for office visits and surgery vary with the severity and complexity
of the problem and are competitive with other regional ear specialists. Fees
will be made available upon request.
We occasionally contract with insurance plans or medical groups; however,
publications or information contained on web pages by these entities are sometimes
out-of-date or inaccurate. When scheduling your first appointment with us,
we will notify you whether or not we participate in your insurance plan. Please
verify this with us at the time of your first appointment.
Patients enrolled in managed care plans should inform us when making appointments.
If you are enrolled in a managed care organization that requires a referral
from a primary care physician, you are responsible for obtaining the referral
and all documentation required by your plan before services are rendered.
You will be required to make payment in full at the time of your exam
if a referral is not obtained.
If there are special financial circumstances, please discuss them with our
office personnel prior to services being rendered.
A Final Note
We appreciate your confidence and will do all in our power to be worthy
of it. We make a special effort to explain everything to you regarding
your condition, medicines, treatment and aftercare. If you ever have questions,
or if something is not clear to you, please do not hesitate to ask.
